Her speech shocked audiences around the world when she was given the boot on the reality show that started it all. And while Susan Hawke may not have won a million, this "Survivor" is ready to look like one. So "Extra" treated her to a makeover, and you won't believe the results.

"I'm 42 years old so I'm hitting the age where things are falling no matter what you do," Hawke admitted before we started. "I want to be like, 'Oh my God, she is sexy!' I want to be hot."

But Sue acknowledged it may take a major overhaul: "I look in the mirror and I'm like, 'Wrinkles, need to maybe iron them out.' The breasts are obvious because I haven't ever even gone through development. And on 'Survivor' I lost 15, 18 pounds easy, but my saddle bags were still there."

Enter "Extra's" makeover team: plastic surgeon Dr. Julio Garcia, fashion expert Lawrence Zarian, colorist to the stars Louis Licari, hair stylist Mel-la Murphy, nail expert Eve Bronner and Mac Senior Makeup Artist Gregory Arlt. And with this tribe of beauty specialists assembled, the makeover began.

"It's a lot of surgeries being done all at the same time," Garcia told us about his approach. "When you look at the whole thing, the package will really be quite a big change for her."

Tim, Susan's husband of 17 years, was fearful about the surgery, but knows his wife is one tough trucker. "I'm okay with it, and I was always against it, but now I'm 40 years old and whatever makes her happy," Tim told us.

After three and a half hours of surgery, both patient and physician were pleased. "Oh wow, it doesn't look bad," Hawke said. "For what I've been through it looks good."

"I think we've changed her in a lot of subtle ways, that overall, the package will be totally different," Garcia observed. "I'm really happy."

But that was just the beginning -- Sue had no idea yet what she was about to go through to become a new person, and only "Extra" caught it all on her exclusive video diary.
